Position Description

Job Summary And Responsibilities

As our Wound Ostomy Nurse, you will connect our hospital directly to the community, delivering specialized in-home care that empowers patients to heal comfortably and effectively.

Every day you will expertly manage in-home patient visits, performing advanced treatments like PICC line care, wound vacs, and infusions, while educating patients and families on complex conditions and treatments.

To be successful in this role you must be an exceptionally organized, independent professional, adept at managing complex schedules, patient care, and technology-driven documentation. Your deep commitment to patient engagement and advocacy will make you a vital defense against declining health, fostering a profound impact on individual well-being.

Essential Functions

  • Demonstrates advanced technical nursing skills and clinical competence in wound care and enterostomal therapy.
  • Utilizes the nursing process of assessment, diagnosis, planning, implementation, and evaluation to ensure quality ostomy and skin care for patients.
  • Ensures continuity of patient care by: Ongoing collaboration with staff regarding patients care during their hospitalization.
  • Participating in discharge planning needs of patients
  • Providing education to patients and families regarding post-hospitalization care.
  • Ensuring home care follow-up needs are met.

Required

Job Requirements

  • Certification of completion of Wound, Ostomy, CWON training program
  • Registered Nurse: CA RN
  • Basic Life Support - CPR BLS

Preferred

  • Acute hospital experience
  • Certified Wound Ostomy Nurse CWON
