Position Description
MEDIX is seeking a dedicated Licensed Vocational Nurse to join their Street Medicine Program. This role involves providing compassionate, community-based healthcare services to individuals experiencing homelessness. The position requires delivering high-quality medical care in non-traditional settings, such as streets, encampments, and shelters, with the goal of bridging healthcare gaps for vulnerable populations.
Job Duties
- Deliver direct patient care under the supervision of a licensed physician or registered nurse (RN).
- Administer medications, treatments, and wound care as prescribed.
- Assist in assessing patient health needs, performing vital sign checks, and documenting findings accurately in the electronic health record (EHR).
- Collaborate with the interdisciplinary street medicine team to develop and implement individualized care plans.
- Educate patients on medication adherence, chronic disease management, and preventative health practices.
- Provide crisis intervention and de-escalation in challenging situations.
- Ensure proper infection control practices while delivering care in various field settings.
- Establish and maintain trust with patients, fostering a safe, non-judgmental environment.
- Participate in outreach efforts, helping locate and engage unhoused individuals in need of medical services.
- Coordinate care and referrals with community organizations and social service providers to address holistic patient needs.
- Maintain and ensure proper inventory of medical supplies and equipment for mobile operations.
- Comply with all organizational policies, procedures, and ethical standards in delivering care.
Qualifications
- Active and unrestricted California Licensed Vocational Nurse (LVN) license.
- Minimum of 1–2 years of nursing experience, preferably in community health, urgent care, or working with underserved populations.
- Current BLS/CPR certification.
- Bilingual English and spanish
- Experience working with individuals experiencing homelessness or behavioral health challenges preferred.
- Valid driver’s license and reliable transportation.
- Ability to stand, walk, and move for extended periods while providing care in various environments, including encampments, shelters, and mobile units.
Schedule:
Full time - 8 hour shifts - 7:30AM/8:00AM (Start time)
