Pride Health is currently seeking candidates for a "RN-Case Manager” position that will provide support to our client's healthcare systems in Greenbrae, CA 94904. This a contract assignment with a high likelihood of extension and a great way to start working with a top-tier healthcare organization!

Shift: Morning (M-F)

Pay Range: $2750 - $3000 Gross Weekly

Overview:

Job Description:

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager. The ideal candidate will have strong experience in Utilization Review and InterQual criteria. As an RN Case Manager, you will be responsible for conducting initial patient assessments, collaborating with multidisciplinary teams, and ensuring the quality of care and appropriate resource utilization. The role requires a solid understanding of acute care hospital settings, EPIC EMR documentation, and the ability to manage complex cases effectively.

Key Responsibilities:

  • Conduct initial assessments for patients to determine appropriate care plans and resource allocation.
  • Perform Utilization Review using InterQual criteria to ensure that services and treatments provided are medically necessary and meet quality standards.
  • Coordinate and manage the care of patients from admission to discharge, ensuring timely and efficient use of hospital resources.
  • Collaborate with physicians, social workers, and other healthcare professionals to develop and implement comprehensive care plans.
  • Ensure proper documentation and coding of patient information in the EPIC EMR system.
  • Monitor patient progress, making adjustments to care plans as necessary.
  • Provide education and support to patients and families regarding treatment plans, discharge procedures, and resources.
  • Advocate for patients while ensuring compliance with insurance and regulatory requirements.

Qualifications:

  • Active California RN License required.
  • Minimum of 2-3 years of acute care hospital experience.
  • Strong experience with Utilization Review and InterQual criteria.
